Medicare Appointment (Enhanced Primary Care Plan)

EPC Plan (Enhanced Primary Care)

The Enhanced Primary Care (EPC) plan, funded by Medicare, enables patients with chronic medical conditions to access a wide range of allied health services. This program aims to improve patient outcomes through coordinated care and personalised treatment plans.


Eligibility for the EPC Plan

Patients with chronic or complex medical conditions, requiring multidisciplinary care, may be eligible for the EPC plan. Conditions include, but are not limited to:

  • Diabetes
  • Cardiovascular disease
  • Chronic respiratory disease
  • Musculoskeletal conditions
  • Mental health conditions


Services Covered

Under the EPC plan, Medicare provides rebates for up to five allied health services per calendar year. These services can include:

  • Podiatry
  • Osteopathy
  • Physiotherapy
  • Dietetics
  • Occupational therapy
  • Psychology


How to Get Started

  1. Consult Your GP: Discuss your eligibility for the EPC plan with your General Practitioner (GP). Your GP will assess your condition and determine if you qualify.
  2. Referral: If eligible, your GP will provide a referral for allied health services under the EPC plan. Have our details ready (Podiatrist and address).
  3. Book the Appointment type you require: Once your form is filled in, signed, dated and received (you can bring in the physical signed copy or have the signed copy emailed - myfoothealth@outlook.com).


Rebate Available On-the-Spot

At the end of your appointment, once you pay the full amount, you can then receive the rebate in the clinic via your debit card. As of 1 July 2024, the rebate amount is $60.35. 

EPC form needs to be filled in, including the practice location and GP signature with date.
