The Enhanced Primary Care (EPC) plan, funded by Medicare, enables patients with chronic medical conditions to access a wide range of allied health services. This program aims to improve patient outcomes through coordinated care and personalised treatment plans.
Patients with chronic or complex medical conditions, requiring multidisciplinary care, may be eligible for the EPC plan. Conditions include, but are not limited to:
Under the EPC plan, Medicare provides rebates for up to five allied health services per calendar year. These services can include:
At the end of your appointment, once you pay the full amount, you can then receive the rebate in the clinic via your debit card. As of 1 July 2024, the rebate amount is $60.35.
EPC form needs to be filled in, including the practice location and GP signature with date.
